Hate to be That Guy, but this one is on the Captain. He didn't maintain VMCA because he pushed up the left engine much too far without a corresponding increase on the right, and likely wasn't prepared for the rudder/aileron input that would be required to combat that. This falls under the "quick hands" concept that aviators try to avoid. Get the thing stable and under control at altitude, then make minimal changes on the descent. That's not to say that I don't understand why it went the way it did or that I think I'd do better, just that he could have and with his experience level probably should have.
