Why did VA's systems fail this Marine?
VA's pharmacy dispensing program has a software mechanism that displays pharmaceutical contraindications, why didn't this work?

the VA has been alot of problems internally. This is not the first time that a service member has died under the VA care. Bottom line, I believe, it's incompitence of the caregivers.
