Collisions were due to human error. Publically, Navy never said any ship's electronic or mechanical system failed.

16 years of war, cuts to the fleet, cuts to maintenance periods, etc... it all adds up to disaster.

FDNF is a hard job but not sure we can blame these collisions on that, Fitz was just a case of a crappy JO that should be charged with 7 counts of manslaughter and assaulting his commanding officer.
