I don't remember ever seeing both the CO & XO relieved, in any service.
Reminds me of that recent Engineer Battalion article where both the Battalion Commander and the CSM were relieved.

SSG Dennis R. While the Skipper is Ultimately Responsible. The XO is the Administrative Head of the Ship so Signs off all the Quals. It's not that Uncommon in the Navy that they both take the Fall.
