Urban warfare is one of the most deadliest forms of warfare SFC (Join to see). It was challenging before the 20th century but the battle of Stalingrad forever changed the image of urban warfare.
I went through MOUT training a few times including at Hammelburg MOUT training area in Germany and Fort Benning. Urban warfare sewer and transportation systems and other underground pathways are incredibly challenging areas if they are integrated into a coordinated defense.

Remember Stalingrad! There on the Volga River the Red Army withstood wave after wave of the best of the Wehrmacht, decimating Gen. Paulus' 6th Army, until surrendering to the Reds in Feb. 1943. The street fighting there made every building a fortress, every high tower a sniper nest, until the Germans bled out as an effective fighting force. Those Nazis became poster children for combat stress or PTSD.

The Army once focused on Military Operations in Urban Terrain (MOUT); it is hard, dangerous and some of the nastiest combat there is...Fallujah comes to mind and it was nowhere near a mega city.
